What information is provided
- Name, license number, license status, license issue date, license expiration date, medical school, medical school attendance dates.
- ACSC/QACSC (controlled substances certificate) number, status, date issued, date of expiration.
- CRNPs/CNMs in collaborative practice with and PAs/AAs registered to physicians.
- Disciplinary and public actions are indicated by a “yes” next to “Public file” and downloadable. License and registration actions are a permanent record. (not currently online)
What information is not provided
- Some information is confidential pursuant to state law:
- Patient complaints.
- Malpractice case information.
- Investigations.
- Full addresses and birth dates are not provided for security reasons.
