To have a written license verification form sent to another state licensing board or other entity, please follow the steps below:

1. Log into your Licensee Gateway account *If you do not already have an account, you must first register. 

2. In the upper right corner, hover over "My Profile"

3. Select "Request License Verification" 

4. Follow the on-screen instructions to submit your request
