Request a Verification

To have a written license verification form sent to another state licensing board or other entity, use either option below.

Log into your Licensee Gateway account *If you do not already have an account, you must first register.
In the upper right corner, hover over "My Profile"
Select "Request License Verification"
Follow the on-screen instructions to submit your request

Find the license you want to send using our licensee search page.
Search for the license using the available fields: name, license type, and license number
Select the licensee's name next to the license that you need verification for
Select the blue "Send to Agency" button at the bottom of the screen
Select the agency/institution that you want to send the verification to from the Select Recipient list
If the desired agency is not found in the list, select Alabama Board of Medical Examiners as the Recipient, and put the email address of the agency you want to receive the verification in the Email CC field. Multiple email addresses can be added to the Email CC field. Make sure to separate email address entries with a comma.
