Changes to the Submission of Covering Physician Requests for Collaborative & Registration Agreements

Covering physician requests for collaborative practice agreements and registration agreements may now be submitted online through the collaborating/supervising physician's Licensee Gateway.

Beginning June 8, 2026, all covering physician requests should be submitted online.

Follow the instructions below to add covering physicians to a collaborative or registration agreement.

Adding a covering physician to a Collaborative Practice Agreement (CRNP/CNM):

Physician must log into their Licensee Gateway account
Once logged in, select the collaborative agreement you wish to add a covering physician on (the box will be green or blue)
On the next screen, select the green icon to the left that states, "Covering Physician Selection"
Follow the on-screen instructions
After this step is complete, the covering physician(s) must log into their Licensee Gateway account to sign the form
Once the covering physician logs in, the pending covering physician request will be on the home page of their Licensee Gateway account under "Applications in Progress"
Select the application, "CP Covering Approval" and follow the on-screen instructions
Covering physician agreements will be processed after the covering physician(s) submits their signature

Adding a covering physician to a Registration Agreement (PA):

Physician must log into their Licensee Gateway account
Once logged in, select "Related Licenses/Registrations" above the listed MD/DO license
Select the registration agreement you wish to add a covering physician on (the box will be green or blue)
On the next screen, select the green icon to the left that states, "Covering Physician Selection"
Follow the on-screen instructions
After this step is complete, the covering physician(s) must log into their Licensee Gateway account to sign the form
Once the covering physician logs in, the pending covering physician request will be on the home page of their Licensee Gateway account under "Applications in Progress"
Select the application, "CP Covering Approval" and follow the on-screen instructions
Covering physician agreements will be processed after the covering physician(s) submits their signature
